An interesting take...

The buzz is that the CBO will show that the new GOP plan will reduce coverage by several millions. Those in the know say that these numbers will not just include those losing insurance... but also include people who went from "comprehensive" coverage to having what might otherwise be known as "catastrophic" coverage. Basically demanding that a so called reduction of coverage is the same thing as losing coverage.

With the House repeal bill under attack by Republican moderates worrying about coverage and conservatives fuming about entitlements and spending, the CBO assessment will matter. It’s widely expected early next week. Hall, in the post for two years, has signaled that his office won't soft-pedal the coverage assessments. If a health plan doesn't have comprehensive benefits, it won't count as coveragewww.politico.obamacare-repeal-congressional-budget-office 

But here is the rub. Currently the CBO is including in their numbers (as they should) all plans, including those that may be comprehensive, but have deductibles so high that they would only really kick in if they ran into a medical issue that many might label "catastrophic".

These are plans that in some case cost people several hundred dollars a month, but only kick in after several thousands of out of pocket expenses are paid first. Most of the people on these plans receive the highest amount of subsidies, as those who do not receive subsidies cannot afford it.

So this begs the question... what's really the difference between how many different things are covered, if you never get to the point of being able to be reimbursed? If given the option, many of these people would choose a much less expensive catastrophic plan. In fact the availability of such plans would lead to more coverage, not less... by almost every available means to measure. But not (according to the CBO) who will insist that only coverage that they deem acceptable can be counted.

This seems to be doubling down on Government overreach and Government stupidity. Government bureaucracy demand that everyone has to purchase the insurance they deem they should have. If an American citizens purchases another form of insurance that isn't what the Government believes that they should have, then another bureaucracy will claim they didn't actually purchase anything.

And people wonder why the 2016 electorate rejected the status quo?

Actually Roger - The CBO numbers always over-inflated the effects of Obamacare. I guarantee you that you can go through both this blog and the old legacy blog...

and not find a single post I wrote that was favorable to anything the CBO stated on Obamacare. I think you would be hard pressed to find anything I ever wrote about the CBO that was positive.

The Congressional Budget Office projects that 14 million people will lose coverage by 2018 under the Republican ObamaCare replacement bill.



The long-awaited analysis from the nonpartisan congressional scorekeeper is sure to shake up the debate over the measure, which is already facing sharp criticism from conservatives and many centrist Republicans.

The GOP bill repeals ObamaCare’s subsidies to buy coverage, replacing them with smaller tax credits, as well as the law’s Medicaid expansion after 2019. Both moves were expected to lead to coverage losses.

Republicans had expected that the CBO would show Americans losing coverage, and preemptively went on the offensive against the nonpartisan agency, whose director, Keith Hall, who was appointed by the GOP.

White House press secretary Sean Spicer last week argued CBO was “way off” in its ObamaCare projections. Our esteemed host took his orders, saluted and stated YES SIR. Seig Heil.

"If you're looking to the CBO for accuracy, you're looking in the wrong place,” he said.

White House budget director Mick Mulvaney, meanwhile, argued Sunday on ABC’s “This Week” that the CBO shouldn’t even try to analyze the bill.

“Sometimes we ask them to do stuff they’re not capable of doing, and estimating the impact of a bill of this size probably isn’t the best use of their time,” he said.

Macroeconomic Effects
Because of the magnitude of its budgetary effects, this legislation is “major legislation,” as defined in the rules of the House of Representatives.

Hence, it triggers the requirement that the cost estimate, to the greatest extent practicable, include the budgetary impact of its macroeconomic effects. However, because of the very short time available to prepare this cost estimate, quantifying and incorporating those macroeconomic effects have not been practicable.
Intergovernmental and Private-Sector Mandates
JCT and CBO have reviewed the provisions of the legislation and determined that they would impose no intergovernmental mandates as defined in the Unfunded Mandates Reform Act (UMRA). JCT and CBO have determined that the legislation would impose private-sector mandates as defined in UMRA. On the basis of information from JCT, CBO estimates the aggregate cost of the mandates would exceed the annual threshold established in UMRA for private-sector mandates ($156 million in 2017, adjusted annually for inflation).
MAJOR PROVISIONS OF THE LEGISLATION
Budgetary effects related to health insurance coverage would stem primarily from the following provisions:


Eliminating penalties associated with the requirements that most people obtain health insurance coverage and that large employers offer their employees coverage that meets specified standards.


Reducing the federal matching rate for adults made eligible for Medicaid by the ACA to equal the rate for other enrollees in the state, beginning in 2020.


Capping the growth in per-enrollee payments for most Medicaid beneficiaries to no more than the medical care component of the consumer price index starting in 2020.

Repealing current-law subsidies for health insurance coverage obtained through the nongroup market—which include refundable tax credits for premium assistance and subsidies to reduce cost-sharing payments—as well as the Basic Health Program, beginning in 2020.


Creating a new refundable tax credit for health insurance coverage purchased through the nongroup market beginning in 2020.


Appropriating funding for grants to states through the Patient and State Stability Fund beginning in 2018.


Relaxing the current-law requirement that prevents insurers from charging older people premiums that are more than three times larger than the premiums charged to younger people in the nongroup and small-group markets. Unless a state sets a different limit, the legislation would allow insurers to charge older people five times more than younger ones, beginning in 2018.


Removing the requirement, beginning in 2020, that insurers who offer plans in the nongroup and small-group markets generally must offer plans that cover at least 60 percent of the cost of covered benefits.


Requiring insurers to apply a 30 percent surcharge on premiums for people who enroll in insurance in the nongroup or small-group markets if they have been uninsured for more than 63 days within the past year. Other parts of the legislation would repeal or delay many of the changes the ACA made to the Internal Revenue Code that were not directly related to the law’s insurance coverage provisions. Repealing the surtax on certain high-income taxpayers’ net investment income;


Repealing the increase in the Hospital Insurance payroll tax rate for certain high-income taxpayers;


Repealing the annual fee on health insurance providers; and


Delaying when the excise tax imposed on some health insurance plans with high premiums would go into effect. In addition, the legislation would make several changes to other health-related programs that would have smaller budgetary effects.

ESTIMATED COST TO THE FEDERAL GOVERNMENT
CBO and JCT estimate that, on net, enacting the legislation would decrease federal deficits by $337 billion over the 2017-2026 period (see Table 1). That change would result from a $1.2 trillion decrease in direct spending, partially offset by an $883 billion reduction in revenues.
BASIS OF ESTIMATE
For this estimate, CBO and JCT assume that the legislation will be enacted by May 2017. Costs and savings are measured relative to CBO’s March 2016 baseline projections, with adjustments for legislation that was enacted after that baseline was produced. The largest budgetary effects would stem from provisions in the recommendations from both committees that would affect insurance coverage. Those provisions, taken together, would reduce projected deficits by $935 billion over the 2017-2026 period. Other provisions would increase deficits by $599 billion, mostly by reducing tax revenues. All told, deficits would be reduced by $337 billion over that period, CBO and JCT estimate. (See Table 2 for the estimated budgetary effects of each major provision.)
Budgetary Effects of Health Insurance Coverage Provisions
The $935 billion in estimated deficit reduction over the 2017-2026 period that would stem from the insurance coverage provisions includes the following amounts (shown in Table 3):


A reduction of $880 billion in federal outlays for Medicaid;


Savings of $673 billion, mostly stemming from the elimination of the ACA’s subsidies for nongroup health insurance—which include refundable tax credits for premium assistance and subsidies to reduce cost-sharing payments—in 2020;


Savings of $70 billion mostly associated with shifts in the mix of taxable and nontaxable compensation resulting from net decreases in the number of people estimated to enroll in employment-based health insurance coverage; and


Savings of $6 billion from the repeal of a tax credit for certain small employers that provide health insurance to their employees.

7 Those decreases would be partially offset by:


A cost of $361 billion for the new tax credit for health insurance established by the legislation in 2020;


A reduction in revenues of $210 billion from eliminating the penalties paid by uninsured people and employers;


An increase in spending of $80 billion for the new Patient and State Stability Fund grant program; and


A net increase in spending of $43 billion under the Medicare program stemming from changes in payments to hospitals that serve a disproportionate share of low-income patients.

Methodology.
The legislation would change the pricing of nongroup insurance and the eligibility for and the amount of subsidies to purchase that insurance. It would also lead to changes in Medicaid eligibility and per capita spending. The legislation’s effects on health insurance coverage would depend in part on how responsive individuals are to changes in the prices, after subsidies, they would have to pay for nongroup insurance; on changes in their eligibility for public coverage; and on their underlying desire for such insurance. Effects on coverage would also stem from how responsive firms are to changes in those post subsidy prices and in the attractiveness of other aspects of nongroup alternatives to employment-based insurance. To capture those complex interactions, CBO uses a microsimulation model to estimate how rates of coverage and sources of insurance would change as a result of alterations in eligibility and subsidies for—and thus the net cost of—various insurance options. Based on survey data, that model incorporates a wide range of information about a representative sample of individuals and families, including their income, employment, health status, and health insurance coverage. The model also incorporates information from the research literature about the responsiveness of individuals and employers to price changes and the responsiveness of individuals to changes in eligibility for public coverage. CBO regularly updates the model so that it incorporates information from the most recent administrative data on insurance coverage and premiums. CBO and JCT use that model—in combination with models of tax revenues, models of Medicaid spending and actions by states, projections of trends in early retirees’ health insurance coverage, and other available information—to inform their estimates of the numbers of people with certain types of coverage and the associated federal budgetary costs.

Effects of Repealing Mandate Penalties.
Eliminating the penalties associated with two requirements, while keeping the requirements themselves in place, would affect insurance coverage in various ways. Those two requirements are that most people obtain health insurance coverage (also called the individual mandate) and that large employers offer their employees health insurance coverage that meets specified standards (also called the employer mandate). Eliminating their associated penalties would reduce federal revenues starting in 2017, but CBO and JCT estimate that doing so would also substantially reduce the number of people with health insurance coverage and, accordingly, would reduce the costs incurred by the federal government in subsidizing some health insurance coverage. The estimated savings stemming from fewer people enrolling in Medicaid, in health insurance obtained through the nongroup market, and in employment-based health insurance coverage would exceed the estimated loss of revenues from eliminating mandate penalties. CBO and JCT estimate that repealing the individual mandate penalties would also result in higher health insurance premiums in the nongroup market after 2017.

Insurers would still be required to provide coverage to any applicant, would not be able to vary premiums to reflect enrollees’ health status or to limit coverage of preexisting medical conditions, and would be limited in how premiums could vary by age. Those features are most attractive to applicants with relatively high expected costs for health care, so CBO and JCT expect that repealing the individual mandate penalties would tend to reduce insurance coverage less among older and less healthy people than among younger and healthier people. Thus, the agencies estimate that repealing those penalties, taken by itself, would increase premiums. Nevertheless, CBO and JCT anticipate that a significant number of relatively healthy people would still purchase insurance in the nongroup market because of the availability of government subsidies.

CBO: 14M more Americans won't have health insurance next year if GOP plan adopted
The Congressional Budget Office released its report on the Republican's health care plan, saying that an estimated 14 million more people will be uninsured next year if the plan becomes law.

The report estimates that by 2018, "14 million more people would be uninsured under the legislation than under current law."

One of the biggest changes between the new health care plan and the Affordable Care Act, commonly known as Obamacare, is that there is no penalty for people who do not sign up for insurance. The CBO states that taking that requirement out is one of the biggest contributing factors to the increase in the number of those uninsured.

"Most of that increase would stem from repealing the penalties associated with the individual mandate. Some of those people would choose not to have insurance because they chose to be covered by insurance under current law only to avoid paying the penalties, and some people would forgo insurance in response to higher premiums," the report states.

The number of those uninsured under the proposed plan only continues to grow over time: 21 million more people uninsured by 2020 than would be under Obamacare. That number climbs to 24 million by 2026, according to the CBO.

"In 2026, an estimated 52 million people would be uninsured, compared with 28 million who would lack insurance that year under current law," the report states.

The GOP plan would decrease the federal deficit by $337 billion between 2017 and 2026.

The CBO is a nonpartisan agency that produces budget and economic projections, along with cost estimates of pending legislation that project how bills would affect federal spending and revenues.

The agency’s report on the GOP's American Health Care Act will give lawmakers and the American people a more complete picture of the GOP plan and what it would do if passed. Depending on the estimates, it could help determine how many members of Congress vote for it – and whether the bill passes the House and Senate.

White House and administration officials started undermining the CBO results days ahead of its release, with press secretary Sean Spicer saying on March 8 that "if you're looking to the CBO for accuracy, you're looking in the wrong place," pointing to the CBO's 2010 reports on the Affordable Care Act.

"Anyone who can do basic math can understand their projections for Obamacare the last time were way, way off the mark,” he said.

Asked if the numbers are legitimate in today's briefing, Spicer said, "All I'm suggesting to you is the numbers they did the last time they did health care were off by more than 50 percent when it came to the number of people insured. That's not my interpretation. That's a fact."

The March 2010 report from the CBO on Obamacare was way off in its estimates of marketplace enrollment. In 2010 the CBO projected that by 2016 21 million people would have health insurance via the ACA marketplaces, but in reality, that number was closer to 12 million.

ObamaCare overhaul faces criticism from both parties

Congressional budget scorekeepers released their highly anticipated analysis of Republicans’ ObamaCare repeal bill late Monday, estimating millions more people would be uninsured under their legislation – though it would reduce the federal deficit.

The Congressional Budget Office predicted 14 million more people would be uninsured in 2018, with that number growing to 24 million by 2026.

The projections are likely to fuel opponents who warn the measure would toss millions of voters off insurance plans. Republican supporters of the repeal plan, however, for days have downplayed the significance of the CBO findings, noting that the same office’s coverage projections for the original Affordable Care Act were off by millions.

Further, President Trump and congressional allies contend the current law is unsustainable.

“It’s imploding,” Trump said in a meeting earlier Monday with Americans the White House described as “victims” of ObamaCare.

Trump vowed that the Republican plan eventually would bring insurance costs “down, down, down.”

The CBO report, compiled along with staff of the Joint Committee on Taxation, determined that the Republicans’ American Health Care Act also would save money for taxpayers. According to the study, it would reduce federal deficits by $337 billion from 2017 to 2026.

“The largest savings would come from reductions in outlays for Medicaid and from the elimination of the Affordable Care Act’s (ACA’s) subsidies for nongroup health insurance. The largest costs would come from repealing many of the changes the ACA made to the Internal Revenue Code—including an increase in the Hospital Insurance payroll tax rate for high-income taxpayers” and other taxes,” the report said.

The report’s estimate on the number of uninsured surely will be used to bolster critics’ case against the plan. Criticism has come from Democrats, Republicans from states that benefit from Obama's law and many corners of the health-care industry.

According to the CBO, the projected loss in insurance coverage is related mostly to a provision repealing penalties associated with the ACA’s requirement to buy insurance.

“Some of those people would choose not to have insurance because they chose to be covered by insurance under current law only to avoid paying the penalties, and some people would forgo insurance in response to higher premiums,” the CBO said.

i know a number of people who are using medi-share and the like and are very satisfied with it. these folks are small business owners, self employed, etc. so i guess you could say that they really are insured. just not in a way the government approves.

of course medi-share and plans like it are christian sponsored. so of course the government disapproves.

now, if you had moose-limb share, the government would be fine with it.

Reports have suggested that the Congressional Budget Office will release their analysis this week of the American Health Care Act, including the all-important forecast of how many Americans will lose coverage. The office’s track record is, well, not very good. CBO vastly overestimated the number of Americans who would enroll on the exchanges and pay the individual mandate penalty. Each year, CBO revised their estimates lower and lower, and still never quite predicted how weak the enrollment would be. This post is not meant as a critique of CBO’s methodology, but rather a frank recognition that even the best experts cannot predict, with any accuracy, the impact of such a massive government program. Indeed, in June 2014, CBO threw in the towel on scoring Obamacare’s complete impacts:

“The provisions that expand insurance coverage established entirely new programs or components of programs that can be isolated and reassessed,” the office wrote.

“In contrast, other provisions of the Affordable Care Act significantly modified existing federal programs and made changes to the Internal Revenue Code.

“Isolating the incremental effects of those provisions on previously existing programs and revenues four years after enactment of the Affordable Care Act is not possible.”
