How do we pay for Medicare for all?

Study: ‘Medicare for all’ projected to cost $32.6 trillion
The idea won broad rank-and-file support after Sanders ran on it in the 2016 Democratic presidential primaries. Looking ahead to the 2020 election, Democrats are debating whether single-payer should be a “litmus test” for national candidates. The Mercatus analysis estimated the 10-year cost of “Medicare for all” from 2022 to 2031, after an initial phase-in. Its findings are similar to those of several independent studies of Sanders’ 2016 plan. Those studies found increases in federal spending over 10 years that ranged from $24.7 trillion to $34.7 trillion. 
So let's split the difference here and round off for the sake of simplicity. Taking an average between the low study that finds a $24.7 trillion ten year price tag and the high study that states it will cost $34.7 trillion, we get $29.7 trillion or approximately $30 trillion over 10 years. That's about $3 trillion dollars a year. That is between four and five times what we spend on our entire military, and about 75% of what we currently spend all together (approx $4 trillion)

So as it stands, we spend approximately $2.5 trillion a year on Mandatory spending (which includes all of our entitlement spending). The "Discretionary spending" (which includes all military spending) is less than half of that number (or approximately $1.2 trillion).

So how could you pay for "another" $3 trillion a year in mandatory entitlement spending? Well here are some ways:
  • Triple our individual income tax (whatever you pay now in taxes, triple it).
  • Increase our Payroll taxes (medicare, medicaid, etc) by approximately 250%.
  • Increase Corporate Income taxes by 10 times.
  • Double all three (income, payroll, corporate). 
Now, of course, you won't hear those solutions from anyone who is actually pushing for a "medicare for all" plan. People would be outraged if their taxes doubled or tripled. Rather supporters of "free stuff" will whitewash the situation by suggesting that the $30 trillion can be paid for through slashes to defense (which in total accounts for about $600-700 billion per year), raising taxes on corporations (which currently brings in just under $300 billion), raising taxes on the rich, and otherwise cutting waste in the approximate $600 billion "non-defense" discretionary spending.

So how to other countries pay for it? Well in places like Germany, people still purchase supplemental insurance, because their universal coverage only covers the bare necessities. In places like Sweden, they close their borders to immigration for the most part, and require those who do enter to pay a substantial citizenship fee. Also, none of those countries have 330 million people to work with, with people pouring over the borders wanting in.

The reality is that if the United States wants universal healthcare, then we have to figure out how to pay for it. Simply stating that other countries do it, isn't even close to an answer.

Congress passed a spending bill in March that funds the government through September; the President has said he won't sign a new spending bill unless his demands are met.

Add the Wall to the deficit. Send the bill to Mexico.

The cost is somewhere between $2.5 and $3.5 trillion a year. Multiple studies have come to these similar conclusions (which is that it will cost somewhere between 7500 and 10500 per person per year). Do you believe that the normal family of four spends 40,000 a year on health insurance and health care?


While your answer (and the answer of other liberals) seems to be... well we should be able to figure it out. The truth is that you cannot figure it out. It's not a math equation that has a reasonable answer. The answer doesn't exist in real world terms.

Which is why your strategy (and the strategy of people like Sanders and Ocasio-Cortez) is to simply not try to figure it out, speak in generalities, and pretend that it's just one of those unimportant details to be figured out later.

I spend a maximum copay of $4,800 per year under the Kaiser Permanente Senior Advantage package. Kaiser is a nonprofit organization with the exception of the doctors portion of the company.

If you take out the profit factors you could reduce the overall cost to the taxpayers.

It's very complicated and I'm not making a joke.

Roger - you do not dispute that it will cost $3 trillion a year. When asked how you will pay for it... you simply decide to go rhetorical. Blame FOX news, blame Trump, or throw some silly response about people dying.

You simply don't have a real answer.



We had a system (prior to Obamacare) where upwards of nine in ten people were happy with their insurance, health care, etc... We ripped up the entire system in order to fix the problem for the 10% who were not happy.

We are now dealing with the consequences of the Government allowing politicians to overrule actuaries and insurance professionals who had been running things effectively for decades (mostly as non-profit organizations).

Why? Because liberals listened to the "rhetoric" that insurance companies were evil, that the system was broken, and that only the Government could fix it. Only they didn't have any real plan to fix it, other than to over-regulate it to the point where nearly everyone is paying more than they did and getting less in return.

Medicare for all is a double down on stupid. That's all it is, Roger.

This time, somebody needs to tells us specifically how the plan works (and how it is paid for) BEFORE you campaign on the rhetoric. Because you might be dumb enough to fall for the "rhetoric" - most Americans are not that dumb. Most Americans will understand that we cannot afford to pay for something that costs three trillion dollars a year.

On Monday, the Mercatus Center at George Mason University released its study on the overall cost of Sanders’s single-payer legislation, the “Medicare for All Act (M4A).” The study found that the “M4A would place unprecedented strain on the federal budget” — to the tune of $32.6 trillion in added cost over the first 10 years of its implementation. The Associated Press noted that “after taking into account current government health care financing, the study estimated that doubling all federal individual and corporate income taxes would not fully cover the additional costs.” Yikes! Study author Charles Blahous writes, “Enacting something like ‘Medicare for All’ would be a transformative change in the size of the federal government.”

Sanders was quick to push back against the study, claiming that the libertarian policy center was being bankrolled by the Koch brothers — as if that undermines the merits of its argument. “This grossly misleading and biased report is the Koch brothers’ response to the growing support in our country for a ‘Medicare for All’ program,” Sanders groused. And yet a spokesman for Sanders admitted that his office has not conducted a cost analysis on the plan. Ideology trumping reality.

But the Mercatus Center’s study isn’t the only one to conclude that Sanders’s plan would create astronomically high taxes to cover the federal costs. Kenneth Thorpe, a former health policy adviser in the Clinton administration and current professor of health policy at Emory University, authored another study that arrived at a similar conclusion. “If you are going to go in this direction, it’s going to cost the federal government $2.5 trillion to $3 trillion a year in terms of spending,” Thorpe stated. “Even though people don’t pay premiums, the tax increases are going to be enormous. There are going to be a lot of people who’ll pay more in taxes than they save on premiums.”

As Democrats continue to veer hard-left, these outright socialist polices like M4A are increasingly becoming the litmus tests candidates must embrace. But Democrats need to be forced to address the actual cost of this socialist ideology and not simply blame the Koch brothers for math they don’t like.

A new report from the libertarian Mercatus Center drew attention as it showed Bernie Sanders' Medicare for All plan would cost the federal government an additional $32.6 trillion over 10 years.
But the Mercatus report also showed that the national health expenditure - the total amount spent on healthcare in the US by the federal government, states, businesses, and individuals - would actually come in below current projections under Sanders' plan.
So while the price tag for the federal government would increase, the total cost of healthcare would go down while also providing healthcare to more than 30 million uninsured Americans.
A new report on Bernie Sanders' Medicare for All plan finds an whopping cost for the federal government. But the price tag may not be as overall eye-catching as it initially seems.

The report, from the libertarian Mercatus Center, showed that Sanders' plan to extend Medicare to all Americans would increase federal healthcare costs by $32.6 trillion between 2022 and 2032, if implemented as written.

Republicans pounced on the number, arguing it showed that such a plan was not feasible.

"$32.6 trillion dollars. That's how much Washington Democrats' single-payer healthcare proposal would cost over 10 years," House Speaker Paul Ryan tweeted. "Even doubling all federal individual and corporate income taxes wouldn't cover this cost. It is just absurd."

But the eye-catching cost for the federal government cost only tells part of the story. The government is only one piece of the health system's puzzle.


The Department of Health and Human Services also measures the total amount spent on healthcare in the US - including the amount paid by states, private citizens, the federal government, businesses, and more. This all-encompassing number is known as the national health expenditure, or NHE.

According to the Mercatus model, total health spending would actually come in approximately $303 billion lower in 2031 than under current projections, with $7.35 trillion going to healthcare that year versus the $7.65 trillion currently expected. Total national health spending would be $2 trillion lower between 2022 and 2031 under the Medicare for All plan, according to the report.

While the price tag for the federal government would increase significantly, decreased spending by other groups would actually lower the total amount of healthcare spending over the same 10-year period. At the same time, the model also assumes that 30 million more people would get access to healthcare, and many people would get more robust services.

The savings would come from a variety of places, such as the government's ability to leverage its bargaining power into lower prescription drug costs and mandating that all healthcare providers take the lower Medicare payment rate.

The study contains assumptions, and there are numerous political and practical concerns in shifting the burden of healthcare payments to the federal government. But based on the Mercatus model, tens of millions of currently uninsured Americans would have access to healthcare and the US as a whole would end up spending less than it is expected to right now.

The construction industry is having a very difficult situation. Young men and women are not seeking jobs in the construction business. Owners are being forced to raise wages and benefits. It's good news for the skilled trade workers but the cost of new housing upwards and is slowing down the housing market.

My friends who are working in construction management are telling me that they can't find enough people to fill good paying jobs.

The carpenters Union is unable to provide sufficient labor. If I wasn't to put on my tools and go to work the wages are now over $40/hour and benefits, including health insurance and pensions.

There is the rub. The federal government is going to spend a lot more money on health care, but the country is going to spend about the same.


Actually, that would likely be untrue, as not every medical procedure would be covered under a single payer. Many procedures (that are part of our overall medical spending) would be considered optional/elective (and not covered) just as they would be if you had private insurance.

So Americans would still pay out of pocket medical expenses, and many would purchase some form of supplemental insurance to cover those types of costs.
