To mask or not to mask... that is the question?

Proof that taking an actual logic class in college was worthless in 2020

So to have it be told, this is another no lose or no win situation. Apparently whether or not there is any statistical evidence that masks work, they work because Fauci and Birx say they do?

Look, I can understand if states that do not have a mask mandate are seeing a spike in cases, that it might be logical or fair to consider this as an indictment of the policy. But if you are seeing a large spike in cases in states that are using a mask mandate, there seems to be the additional argument that this "also" suggests that mask mandates are working?

It's all a little like Paul Krugman economic reasoning. The reason why Keynesian economic policy has never worked, no matter how many times it's tried isn't an indictment on Keynesian economic policy. It's just prove that we have never gone "far enough" to make it work.

But I digress.

Let's face the facts. This virus has at one time or the other spiked in different areas of the country. It was originally a coastal thing, then it became a sun belt state issue, and now the worst of the virus appears to be crawling up the country into the midwest. That seems to be more of a natural course of events, rather than something controlled by how portions of the country are handling things.

Let's take Montana, Wyoming, and Idaho which sit right next to each other. Montana has a full mask mandate, while Wyoming and Idaho only has portions of the state under a mask mandate. Montana is currently seeing 55 cases per 100K over the past 7 days, Idaho is seeing 41 cases per 100K, while Wyoming is seeing 35 cases per 100K. If the mask mandate is supposed to be the virus killer, why is Montana not seeing less cases than their neighbors? Only North Dakota (which borders them on the east) is seeing more cases?

To be clear, there is currently a full 100% mask mandate effective in States that affects over 238 million Americans or about 75% of the country. The rest of the country (with the exception of the 4 million who live in South Dakota and Utah) are in partial mask mandates, generally requiring them in most larger cities, most places of business, and requiring them for any employee who works in public. So it would likely be fair to say that upwards of 90% of our country (or possibly more) is under a strict mask mandate currently and 99% of the country is under some mask mandate.

Yet our cases continue to spike nationally in spite of nearly every American putting on masks?

The reality (as much as this hurts some) is that we cannot blame South Dakota or Utah for a spike in cases in Wisconsin ,Minnesota or Illinois. Nor can we really blame North Dakota or Wyoming and their few hundred cases a day for the national average shooting up. The virus appears to be gaining more steam everywhere. Maybe the mask mandates are helping to keep the virus from being worse, but that is a difficult thing to "prove" at this point.

I hate to keep beating the dead horse here, but if you look at Sweden (as your example of what not to do according to Fauci and Birx) they have not yet seen a second wave, much less the third wave that we are experiencing now. Yesterday the entire country of Sweden reported under 700 new cases and not a single new death. They have seen less than 200 dead over the past four months. Think about that... less than 200 since July 21st? That is a bad day for California, Texas or Florida.

We are doing it right... as we head into wave three with an average of around 700 deaths per day and 99% of our country on mask mandates. Sweden who according to Fauci doesn't even have a strategy, has had no secondary waves, are averaging less than 2 deaths per day, and are under no mask mandate.

Perhaps someone can explain the logic to me?

Because this was never taught in logic 101.

Wearing cloth masks can help prevent people infected with the virus that causes COVID-19 from spreading the virus. Make sure your cloth mask: fits snugly but comfortably against the side of the face, completely covers the nose and mouth, is secured with ties or ear loops, includes multiple layers of fabric, allows for breathing without restriction, and can be laundered and machine dried without damage or change to shape. zero, eh alky? as of 10/12/20 -

North Dakota’s hospitals have 282 beds open and able to be staffed out of 1,806 total hospital beds, according to daily data sent to the North Dakota Department of Health.

Every day around 3:00 p.m., the state’s hospitals and medical centers provide current information on available beds at their institutions — both ICU and non-ICU.

That information is used for a variety of purposes, including the state health department’s daily “COVID-19 Status Report” web page.

According to the current report, North Dakota hospitals as of October 12 have 29 available ICU beds that can be staffed and 253 available non-ICU beds that can be staffed.

Anti-maskers say being forced to wear a face mask in public violates their constitutional rights. They also claim that wearing a mask does not protect from the virus and could do more harm than wearing one. And some of them believe that the coronavirus (“the scamdemic”) is also a hoax.

Find me a mask mandate state (with a population of 10 million where 92% are urban citizens) where they have seen "less" that 200 deaths in the last four months.

WTF are you talking about?

Every domestic commercial airline has announced that passengers and crew members are required to wear face coverings throughout check-in, boarding and the flight itself.

Although these policies have been in place for months, airlines are tightening enforcement and implementing consequences if you refuse, according to the Airlines for America industry trade organization. Passengers without masks may be denied boarding. Some airlines, including United, are taking it a step further by suspending noncompliant passengers from future travel as well.

COVID-19: How much protection do face masks offer?

Get answers to your questions about face masks, including how to use them properly.

By Mayo Clinic Staff

Can face masks help slow the spread of the coronavirus (SARS-CoV-2) that causes COVID-19? Yes, face masks combined with other preventive measures, such as frequent hand-washing and social distancing, help slow the spread of the virus.

So why weren't face masks recommended at the start of the pandemic? At that time, experts didn't know the extent to which people with COVID-19 could spread the virus before symptoms appeared. Nor was it known that some people have COVID-19 but don't have any symptoms. Both groups can unknowingly spread the virus to others.

These discoveries led public health groups to do an about-face on face masks. The World Health Organization and the U.S. Centers for Disease Control and Prevention (CDC) now include face masks in their recommendations for slowing the spread of the virus. The CDC recommends cloth face masks for the public and not the surgical and N95 masks needed by health care providers.

COVID-19 Is Ravaging Wisconsin, And Wisconsinites Still Aren't Staying Home

For the first nine months of the year, Kevin Kopplin saw exactly one person wearing a mask when he showed up at the door with their pizza delivery.

Kopplin, who delivers for two restaurants in Watertown, said he's been frustrated by the way people in his area have disregarded the statewide mask mandate and other measures aimed at slowing the spread of COVID-19. The one time a customer did wear a mask to interact with him, he made a point of thanking her for it.

Then on Oct. 2, President Donald Trump's COVID-19 diagnosis became national news. Wisconsin's cases had been spiking for a month, and hospitals across the state were saying they were reaching a crisis point. That night, Kopplin saw five customers wearing masks.

HERE'S A LITTLE RESEARCH FOR YOU, CH:

Study: COVID-19 transmission risk on airplanes 'virtually non-existent' when passengers wear masks

Thursday, October 15, 2020 10:31AM

United Airlines says the risk of COVID-19 exposure onboard its aircraft is "virtually non-existent" after a new study finds that when masks are worn there is only a 0.003% chance particles from a passenger can enter the passenger's breathing space who is sitting beside them.

NEW YORK -- A new study released Thursday shows that the risk of COVID-19 transmission is "virtually non-existent" during air travel when passengers wear masks.

The study, conducted by the Department of Defense in partnership with United Airlines, found that when seated with a mask on, "only 0.003% of particles actually made their way into another passenger's breathing zone." The results have yet to be peer-reviewed.

Researchers conducted more than 300 tests over 38 hours of flight time and 45 hours of ground testing during a six-month period.

Each test used a mannequin equipped with an aerosol generator that could mimic breathing and coughing. The generator released 180 million particles -- equivalent to the number produced by thousands of coughs -- with the mannequin's mask on and off.

More than 40 sensors throughout the plane detected the droplets, representing other passengers who could potentially come in contact with the particles emitted.

The study revealed that masks drastically limit the rate of transmission when a passenger is seated. Researchers, however, did not attempt to replicate what would happen if an infected person stands up or moves throughout a cabin.

"I'm not standing here telling people that I know exactly what they should do. What I am telling people is if you are inclined to travel or thinking about air travel, there is a reason today, based on this independent study, that you can feel confident that you can travel safely," said Josh Earnest, CCO of United.

The DoD study reflected similar earlier studies that found that an airplane's unique airflow helps minimize risk. On a plane, air flows down -- not front to back -- helping limit person-to-person transmission. High-efficiency particulate air filters also eliminate 99.9% of airborne particles, including viruses, every two to three minutes.

"This study is confirming that you know among all the different places where one can get infected, flights might actually be one of the safest places to be," said Dr. Rajat Mittal, a professor of mechanical engineering at John Hopkins University.

Yet travelers should remember that air travel involves more than a plane.

As more people start traveling for the holidays, airports will start to get busier.

Experts say it's crucial to wear a mask while walking into an airport, going through security, boarding the plane, sitting through the flight and arriving at the destination.

The study has a few more caveats: Researchers only used a surgical mask and simulated what would happen if only one sick passenger was on a full plane. It also does not account for eating and drinking, which allows travelers to take off their masks briefly.

Montana is doing worse than every border state but North Dakota.... in spite of having stricter mandates than Wyoming and Idaho.

For those of you who may be tired of this mask nonsense, here is an article I just submitted to our local paper:
_________
From the Pulpit
James Boswell
______

In ancient Israel, the prophet Amos strongly condemned the wealthy who took advantage of the poor.

In our own time, political leaders beholden to wealthy backers are trying to distract us from the great robbery being committed against us all. They do this by trying to make us think our worst enemies are terrorists, illegal immigrants, people of other races and ethnicities, and even the poor. They say, “Just look at all those terrible people! See how they cause violence and sow disharmony among you by destroying your inner cities, endangering your suburbs, and taking advantage of the welfare system. They are your greatest enemies, a threat to you and your families.”

In saying this, they attempt to divert attention from our true enemies, the wealthier classes who are committing a far greater, if more subtle, act of violence against us all. By adding to the enormous wealth disparity of our nation, they deprive us of what government was intended to provide, the common welfare. They want us to consider a few instances of violence and looting as worse than the far greater looting perpetrated by the multinational corporate executors, investors, and board members who cynically exploit and steal from all of us.

While encouraging us to regard the poor, non-whites, and immigrants as our greatest enemies, these large-scale looters use government to protect their own selfish economic interests. By appealing to society’s discomfort with different races, immigration statuses, and sexual preferences, they hope to keep us from uniting against our far worse enemies, the outrageously privileged few who not only threaten but cynically exploit and rob our entire society by keeping the economy humming along in ways that are good for Wall Street but not for our streets, and by passing laws and appointing judiciaries intended to support tax cuts for the wealthy, deregulation of corporations, and elimination of environmental protections.

In doing this, they hope to prevent us from demanding government that really is “of, by, and for the people,” government that works for the good of all by providing upward mobility through progressive taxes, protection of pro-labor laws, and better health and economic possibilities for everyone.

All this, perhaps, a modern Amos might say to us today.

You obviously have no clue what I stated.

This isn't about Fauci or Birx...

It's about the circular logic that no matter what happens the left considered it proof that we need more draconian measures.

(cases go up we need to issue more mandates - cases go down, then mandates are working and we need to keep them in place)

Reality Reverend.

Science is all about gathering data and processing it.

Science (as it has always been taught in school) is not about opinions.

Currently the statistics show that the United States has 99% of it's population under some form of mask mandate, and over 75% of the country is in strict mandates that require them for everyone in nearly every public situation. Even in Utah and South Dakota (where mandates are not in place) masking is recommended. Utah has no mandate, but provides free masks in most public places and most people still wear them.

Those are mandates that started cropping up as the "second wave" started to hit.

Yet here we are... weeks and months into a national mask mandate, and cases continue to grow. Excuse people for not necessary accepting the opinions and words of "experts" who continue to pretend that our problem is too little control.

Meanwhile Sweden (10 million people 92% of whom live in 2% of the land - making their population actual denser than most US States) has seen no second or third wave and have only lost 200 people over the past four months. They have had no lock down or mask mandates.

So allow for some of this to be looked at objectively and ask yourself why we believe (in lock step) what we believe?

Fauci and Birx are really one and the same. They've been attached at the hip and Fauci is Birx's mentor.

In reality if they are represented in a balanced decision only one of them should be counted. But I digress

Fauci is caught in a bald-faced lie

The CDC has lots of branches and publications. One of them is the Journal of Emerging Infectious Diseases. It is dedicated to “Fast and broad dissemination of reliable information on emerging infectious diseases.” One would expect Dr. Fauci to both be aware of the Journal, and up to date on its contents. So when he spoke with Dr. Ashton in July, we would expect that he should have read the May 2020 issue of Emerging Infectious Diseases. In it we find, “Nonpharmaceutical Measures for Pandemic Influenza in Nonhealthcare Settings—Personal Protective and Environmental Measures.”

This paper from three months before the interview reviews “the evidence base on the effectiveness of nonpharmaceutical personal protective measures and environmental hygiene measures in nonhealthcare settings … in pandemic plans. The “evidence base” means “everything that is known.”

The authors go on to say that they are looking at handwashing and mask wearing as possible ways to prevent transmission of H1N1 (swine) flu. And before you start yelling, swine flu and Wuhan flu are transmitted the same way. So whatever works (or doesn’t work) for one will work (or not work, as the case may be) for the other.

Handwashing and hand sanitizer were bigtime early recommendations by Fauci and friends as ways to prevent you from getting Wuhan flu. We had massive runs on hand sanitizer, with lots of companies figuring out how to get a piece of that suddenly huge pie. We even had some versions pulled off the market because they weren’t safe.

Originally Fauci and the WHO told us that masks weren’t useful for us and should be saved for medical personnel who needed them. Now Fauci tells us they are essential. His excuse is that he was originally trying to prevent a run on masks. The simpler explanation is that he was lying either at the beginning or the end. But let’s let that slide. The paper in EID didn’t just looked at both hand washing and masks. Without further ado, we should “follow the science” and look at the scientific paper.

First, let’s recall that randomized controlled trials are considered the “gold standard.” They allow an investigator to reach firm, useful conclusions. These investigators looked at fourteen – count ‘em – fourteen RCTs in a “meta-analysis.” Evidence from these RCTs “did not support a substantial effect on transmission of laboratory-confirmed influenza.”

Translation: You can wash your hands all you want or take a bath in hand sanitizer. Or not. You can wear any mask you want. Or not. It’s not going to matter. It won’t make any difference on whether you get Wuhan flu or not. That’s the science.

But that’s not all. Fauci told MarketWatch on July 24 that “masks and respirators reduced the risk of infection by anywhere from 78 percent to 85 percent.” He cited a paper in Lancet to bolster his assertion. But he misrepresented the paper. One key factor is that the authors reviewed 172 observational and 44 comparative studies, but not one RCT. Not one. Further, their search “did not retrieve any systematic review of information on physical distancing, face masks, or eye protection to prevent transmission…”

In short, the paper Fauci cited didn’t have any proof relevant to the question. Had Fauci read past the numbers that looked like what he wanted to see, he’d have realized that it was worthless for forming policy.

So? I don't dispute that Fauci and Birx make these claims.

That being said, they are epidemiologists, and by definition their job is to study the affects and spread of the virus in an objective manner and provide us with the data and recommendations based on that data.

I would feel a whole lot more confident in what Fauci might say if he backed it with tangible data, showing us examples of where mask mandates are working to decrease the spread... rather than just telling us that it works, while states in full mandate mode continue to see their cases grow.

Some of us demand more than some letters behind the name of a person providing an opinion. Considering the best of scientists get almost nothing correct in their first attempts at proving their theories, demanding that every opinion is gospel is sort of not a very scientific approach.

Instead, based on no meaningful “science” whatsoever—but rather guesswork and computer modeling of the crudest kind—most governments adopted the Chinese ant-farm approach, treating their citizens as captives, muzzling them, and charging them with crimes should the comrades step out of line.

This way is unbecoming to the free peoples of the West, an insult to personal sovereignty, a flagrant violation of both the law and the social compact, and an act of hostile aggression against the citizens of the democracies.

Having unwisely acceded to this flagrant bait and switch, Americans and others now find that the goal posts have been moved from last Easter to some indefinite future in which either a vaccine will be found against a disease that kills almost nobody, or the virus itself is eradicated—a fool’s errand if there ever was one.

The commissars have also decreed that it’s not the number of fatalities that now matters, but the number of infections—“cases,” to use the scare term—with the added proviso that even one “case” is unacceptable.

We’ve also swallowed the misrepresentation that the more people we test the closer to liberation day we move because we can now contact-trace the infections to their sources, when in fact the more people who get infected the better it is for our health as a society.

But the governments don’t really care about any of this. Covid-19 is apparently the Worst Thing of All Time and must be dealt with on an immediate draconian basis as if it were World War Three. They’ve thoroughly frightened the population into believing that no price to pay is too great for us to wreck our way of life and our essential humanity, and then to thank—and pay—them for it.

What’s Different

In previous pandemics, the bug was allowed to burn its way through the body politic, its toll just part of the cost of being human. So what’s different about this seasonal malady?

For one thing, it’s occurring in an election year, at the start of which the American economy was firing on all cylinders in a way it hasn’t in decades. When President Trump’s political enemies realized the Covid shutdown could be wielded as a wrecker’s ball against the American and global economies, they jumped at the chance.

For another, a society that has moved from “pay any price, bear any burden” to “if it saves just one life” is no longer capable of facing or dealing with reality. With atheism ascendant, the extension of life (except for that of the unborn, of course) has become the cardinal social virtue.

In the absence of an afterlife, there is literally nothing worth dying for. Saving just one life, however, is meaningless in the scheme of things. Risking and perhaps sacrificing your own existence for the preservation of society and the future of others has become unthinkable.

For a third, governments now have a taste for it, and plan to extend the punishment indefinitely by deeming it the solution to another imaginary threat—“climate change.”

Almost from the start, the Left understood that if they could make the link between consequences of the shutdown and the decline in “carbon emissions”—something that has little or nothing to do with the Earth’s natural cycles of warming and cooling—they could keep the scam going forever.

Grisham says the first lady has decided not to accompany President Donald Trump to a campaign rally Tuesday evening in Erie, Pennsylvania, out of an abundance of caution.

Mrs. Trump’s last public appearance was during the Sept. 29 presidential debate in Ohio. She and the Republican president received their positive test results in early October.

Michael Steele. A nego Republican

Rather than binding up the nation’s wounds, Trump exacerbates division. Rather than standing up to the world’s dictators, Trump cravenly seeks the favor of thugs. Rather than fostering free enterprise, Trump embraces economic principles not only outdated in Lincoln’s time, but made even worse today by a leader who lost close to a billion dollars in a single year running a casino. Rather than seeking to build on the legacy of the Republican Party’s founders, of which Trump is surely ignorant, Trump has posited a single purpose for the GOP — the celebration of him.

Consequently, America has watched as the Republican Party stopped pursuing its animating principles of freedom and opportunity. It has given up its voice on things that mattered and instead bent the arc of the party towards the baser motives of one man, who is neither a Republican nor a conservative.

Related

Many of the grievances that Trump has exploited are legitimate and should be addressed — but responsibly, not demagogically. Much of middle America is, in fact, viewed by coastal elites with a mixture of condescension and contempt. Educational disparities do afflict rural and low-income communities. Economic mobility is impeded by a wide range of factors, especially race.

OPINIONTab was the drink of my generation. Here's why I'm not mourning its demise.

In the 2010 election cycle, I inherited a deeply divided party in which the base rightly, felt ignored by a party establishment indifferent to its concerns. My top priority as party chair was to harness grassroots energy and electoral heft, not through grievance mongering or by highlighting divisions, but by supporting an opportunity agenda implemented with fiscal discipline. By emphasizing unity and a positive message of opportunity and growth, we thrived politically.

But that was then. And I long to restore that trust with the American people. However, what many inside and outside the GOP fail to grasp now is that our lack of standing with the American people is not the fault of our ideals or the principles we espouse, but rather due to our failure to stand up against the arrogance of power and the erosion of our principles.

What many inside and outside the GOP fail to understand now is our lack of standing with the American people is not the fault of our ideals or the principles we espouse

I, of course, disagree with Biden on many issues and policies, sometimes vigorously; and it is my fervent hope that he will pursue policies that will help our country heal. But this election is not about those issues or policies. Rather, it is about the course of a nation and the character of her people reflected in the leader they choose. I am asking my fellow Americans to consider what is in your best interests, and not Donald Trump’s.

In 1860, the Democratic nominee Stephen Douglas, anticipating a Lincoln victory, spent the campaign’s final months traveling throughout the South, calling for unity. After Lincoln’s win, Douglas conspicuously demonstrated that, notwithstanding their political differences, he was part of the loyal opposition, and strongly supported the incoming president’s efforts to bring the country together. Above all, he chose to do what was best for the country.

I'm still wondering if the Republican party can survive

JAMA editorial reviews latest science, while case study shows masks prevented COVID spread

Press Release

For Immediate Release: Tuesday, July 14, 2020
Contact: Media Relations
(404) 639-3286

Americans are increasingly adopting the use of cloth face masks to slow the spread of COVID-19, and the latest science may convince even more to do so.

In an editorial published today in the Journal of the American Medical Association (JAMA), CDC reviewed the latest science and affirms that cloth face coverings are a critical tool in the fight against COVID-19 that could reduce the spread of the disease, particularly when used universally within communities. There is increasing evidence that cloth face coverings help prevent people who have COVID-19 from spreading the virus to others.

“We are not defenseless against COVID-19,” said CDC Director Dr. Robert R. Redfield. “Cloth face coverings are one of the most powerful weapons we have to slow and stop the spread of the virus – particularly when used universally within a community setting. All Americans have a responsibility to protect themselves, their families, and their communities.”

This review included two case studies out today, one from JAMA, showing that adherence to universal masking policies reduced SARS-CoV-2 transmission within a Boston hospital system, and one from CDC’s Morbidity and Mortality Weekly Report (MMWR), showing that wearing a mask prevented the spread of infection from two hair stylists to their customers in Missouri.

Additional data in today’s MMWR showed that immediately after the White House Coronavirus Task Force and CDC advised Americans to wear cloth face coverings when leaving home, the proportion of U.S. adults who chose to do so increased, with 3 in 4 reporting they had adopted the recommendation in a national internet survey.

The results of the Missouri case study provide further evidence on the benefits of wearing a cloth face covering. The investigation focused on two hair stylists — infected with and having symptoms of COVID-19 — whose salon policy followed a local ordinance requiring cloth face coverings for all employees and patrons. The investigators found that none of the stylists’ 139 clients or secondary contacts became ill, and all 67 clients who volunteered to be tested showed no sign of infection.

The finding adds to a growing body of evidence that cloth face coverings provide source control – that is, they help prevent the person wearing the mask from spreading COVID-19 to others. The main protection individuals gain from masking occurs when others in their communities also wear face coverings.

The damn government is a deep state conspiracy against the President and Thecoldheartedtruth!

The UK

The growing evidence on lockdown deaths

That the lockdown had a terrible impact on the nation’s health — in ways other than just Covid-19 — is becoming clearer by the day. But just how bad was it?

According to a study by the London School of Hygiene and Tropical Medicine, delayed and cancelled breast cancer treatments will cause between 281 and 344 additional deaths. For colorectal cancer, there were an extra 1,445 to 1,563 deaths, lung cancer an additional 1,235 to 1372 deaths and 330 to 342 more oesophagal cancer deaths.

A University of Leeds study estimated that there have already been an extra 2,085 deaths from heart disease and stroke as a result of people not accessing timely medical help. A study by the University Hospital of Northern Tees reveals that the number of endoscopies — used to investigate and diagnose bowel cancer — fell to just 12 per cent of their normal level between 24 March and 31 May

The National Blood and Transplant Service looked at the period between 23 March and 10 May and found that, compared with the same period in 2019, the number of organ donors fell by 66 per cent and the number of transplants fell by 68 per cent. This year, 87 people died while waiting for an organ transplant, compared with 47 last year.

According to the ONS, an extra 25,472 people have died at home than would otherwise be expected from the average past five years — some of them, no doubt, would have died even if they had reached hospital, but not all. Meanwhile, the NSPCC has reported that calls to its helpline averaged 8,287 in May compared with 5,593 in early March, as children were shut away at home with their abusers.

These are just a few of the effects of lockdown, and of the poor messaging that led to many people failing to seek medical attention. These numbers will eventually have to be balanced against the lives potentially saved by the lockdown.

And these are much younger than the ones that may have died of Covid

I was speaking with my neighbor the other day, whose age makes him both wise and at high risk should he contract COVID-19. I was wearing a mask — he was not. “Yeah, I don’t do that,” he said. I thought, “He must not understand how the COVID-19 virus is transmitted, because if he did, I think he would wear a mask.”

Dr. Malcolm Butler

Chelan-Douglas Health District health officer

He went on to say, “The science around this changes all of the time, so who are you supposed to trust?” As a scientist, I am OK with some uncertainty as we learn more about diseases. For instance, it was believed that ulcers were caused by stress — now we know that they are largely an infectious disease. It’s the nature of scientific discovery that our “mental model” of how the world works evolves over time. The nature of how we understand COVID-19 is continuously evolving, which is normal and correct — and clearly frustrating.

Coldheartedtruth Teller said...

When the pandemic began, scientists believed it was similar to influenza. We know a great deal about influenza. The last great pandemic involved influenza. It was a natural place to start. Influenza is spread by droplets, and COVID-19 is spread by droplets, so we deployed influenza-type strategies of hand washing and hygiene, wiping down surfaces, social distancing, etc. When that wasn’t enough, we used more draconian measures that included stay home orders and closing all but essential businesses.

Now we understand how COVID-19 is not like influenza; let me explain.

COVID-19 seems to fit the “eighty-twenty rule” of biology. 80% of the disease is caused by 20% of the transmission. Here’s how that 20% works with this virus.

Imagine a keyhole, and only a specific shape can fit through that hole. The COVID-19 virus has a specific shape and must land on a specific keyhole to pass through the lining of your nose, mouth, or lungs and into your bloodstream. These keyholes are spread out randomly. So, it takes multiple hits for a virus to infect you. If you are peppered with enough virus, eventually one will land on the keyhole, pass through and cause an infection. The total volume of virus that peppers your system is the key. How much time you are in contact with an infected person and the time sharing their air determines if you will contract COVID-19.

Both COVID-19 and influenza are transmitted via droplets. But unlike influenza, the droplets that remain suspended in the air are what matter most. Influenza is all about big droplets that land on surfaces, which we touch, and then carry up to our mouths, or eyes. COVID-19 is about small droplets that remain floating in the air that we share.

Imagine your tonsils as the breeding ground of the COVID-19 virus. Anything that pushes air past the tonsils will collect and expel virus. Singing, yelling, breathing hard, coughing, all will generate large droplets filled with virus. Talking or just breathing release small virus-containing droplets. The big droplets contain lots of virus but fall to surfaces within 6 feet — and yes, you could contract the virus from those surfaces, but it is difficult. Very small droplets float around for a long time, but don’t contain many viruses, and aren’t that infectious.

Coldheartedtruth Teller said...

This is why wearing a mask is so important. I must confess that at first the whole universal masking thing made no sense to me. As a physician I have been trained to use a mask in a very specific way to avoid transmitting something like tuberculosis from one patient room to another when working in a hospital. Universal masking violates my medical training. So how could it work?

In China and Italy and New York City — all of the hotspots — the curves finally started to bend only after universal masking was imposed. Why?

Masks, even cloth masks, retain the biggest droplets and those nasty medium sized droplets. Only the small droplets that aren’t very infectious can get through. When an infected person wears a mask, and remember that you are most infectious before you even start to feel sick, the total volume of virus floating around in the air that we share is dramatically reduced. Because 80% of infections come from droplets floating around in the air, the simple act of wearing a mask is enough to stop the pandemic spread. How I wish we had known that in March.

Coldheartedtruth Teller said...

It turns out that in COVID-19, masking is the next best thing to a vaccine. And probably nobody reading this will receive a vaccine before June of 2021.
