Quinnipiac’s new poll gave the Democrats a 10-point lead, lower than other recent polls from the pollsters but hardly bad news. Funny how "unnamed candidate" always polls higher than when a candidate is named, isn't it?
In that case, it's basically an approval or disapproval of the named candidate.
Unnamed, or "generic" GOP candidate, doesn't have any baggage, history or other negatives. Any real candidate will have negatives as soon as they're known well enough, and you'll almost never perfectly agree with them.
Head to head, or generic vs generic, are the right ways to poll it.

Heitkamp polling at 43 against Cramer is not good news for her. He (Cramer) is polling ahead in other polling (49-44 NRSC).
While I don't necessarily agree that every incumbent polling under 50% is in big trouble. I do believe that a Democrat incumbent in a deep red state that is only polling at 44-43% right now... is in big trouble.
Cramer was the first choice for Republicans, and is a well known commodity. It's probably his race to lose.
